Caregiver Support Model: Development and Validation Project

Introduction

Our agency is funded by Simon K. Y. Lee Elderly Fund to run a 14 months “Caregiver Support Model: Development and Validation Project” from November 2021 to December 2022 at Kowloon West District. With collaboration with City University of Hong Kong, the project adopts a scientific, comprehensive and validated working model of serving caregivers of the elderly with the purposes to strengthen their personal social capital so that they can perform the caregiver roles more effectively as well as release their burden. 

Caregivers aged

• 21 years old or above caregivers who provide unpaid assistance to frail elders aged 60 or above 

Service Contents

• Individual Casework and Service Plan for caregivers

Service Hours

Mondays to Saturdays	8:30a.m. - 1:00p.m.
	2:00p.m. - 5:30p.m
Public Holidays & Sunday	Closed

 

Service Application

• Applicants can contact project staff for enquiry via phone or visit our project during service hours.
• The project accepts referrals from other organisations.
